Reserve your flu shots now

Free flu shots will be available on campus on the morning of Wednesday, Oct. 10.
The school health office says the shots will be available to members of all major health plans on Oahu, except Kaiser Permanente.
Reservations are recommended
Here are the details:

HCC FLU SHOT CLINIC

DATE: Wednesday, October 10, 2018

TIME: 10:15am to 1pm (close promptly at 1:15pm)

PLACE: Cafeteria – Mauka side of Bldg 4

WHO: All Welcome – Students, Employees, Friends, & Family

               Also, Volunteers are welcome to come help                

1. BE SURE to MAKE A RESERVATION either ONLINE at
   https://preview.tinyurl.com/2018-HCC-Flu-Shot-Clinic or email hcchlth@hawaii.edu (with your name, cellphone/office phone #, approximate time you are coming, type of insurance) –OR- pick up a HARDCOPY application at info tables outside Library, Health Office, and Bookstore.
2. Check NOW! HAVE your INSURANCE CARD??? IF NOT, call the insurance company to request a new card so you can get it in time.
3. On WEDNESDAY, OCTOBER 10, come between 10:15am and 1pm to the Cafeteria and bring the following:
   • Participating insurance card
     • Government photo ID
   • Subscriber’s name & birthdate
     • Optional – completed Pharmacare Consent Form – may access via http://www.honolulu.hawaii.edu/sites/www2.honolulu.hawaii.edu/files/health-pharmacare-consent.pdf
       Members of the following plans are eligible: Aetna, Aloha Care Advantage Plus, Hawaii Laborers Union, HMA, HMAA, HMN, HMSA, HMSA65C+, HMSA Akamai, HMSA Quest, Medicare Part B, Pacific Administrators, Teamsters, and UHA members.
